For many of the earlier years of the AIDS Epidemic those of us working in HIV believed that the end of the disease would come through the discovery of a cure. Now it seems clear that “The concept of U=U (undetectable equals untransmittable) is the foundation of being able to end the epidemic” (Fauci, NIAID, July 2019). According the CDC, “For persons who achieve and maintain viral suppression, there is effectively no risk of transmitting HIV to their HIV-negative sexual partner. This translates to an effectiveness estimate of 100% for taking ART [antiretroviral therapy] regularly as prescribed and achieving and maintaining viral suppression. Effectiveness is lower, and there is a risk of transmitting HIV, when persons do not take ART as prescribed or stop taking ART, if viral suppression is not achieved, or if viral suppression is not maintained” (2018).
Retaining patients in treatment, however, can be a challenging task, due to such barriers as poverty, homelessness, discrimination, mental illness, problematic substance use, traumatic events, and stigma (Whetton, et al, 2008; Gardner et al, 2011; Kulkarni, 2012). A warm, welcoming clinic environment (particularly to MSM and people of color), the use of Motivational Interviewing spirit and techniques (Miller & Rollnick, 1981, 2005, 2012), a strengths-based attitude toward those we serve (Rapp et al., 2009), and evidence based skills-building tools for HIV patients can build motivation, increase self-efficacy, and have demonstrated effectiveness in enhancing retention of HIV-infected patients in care and promoting medication adherence (Marks, et al., 2010; Thompson et al 2012; Corwin & Bradley-Springer, 2013; CDC “Treatment Works” campaign, 2014; U=U campaign, 2016; Marks, Corwin et al., 2018)

2019 NCSWH Pre-Conference Institute (May 22, 2019): Retention in Care and Medication Adherence: Using Motivational Interviewing, Strengths Based Messaging, and Evidence Based Tools
Marla A. Corwin is a Licensed Clinical Social Worker and a Certified Addictions Counselor, Senior Level. She is a retired faculty member of 25 years at the University of Colorado’s School of Medicine and College of Nursing, with 15 years as a Behavioral Health Specialist for the AIDS Education and Training Center. Marla designs behavioral health education materials and provides training on Motivational Interviewing and Strengths-Based Work with Patients, Retention in Care and Medication Adherence, Patient-Provider Communication, Risk Reduction and Disease Prevention, Sexual History Taking, Suicide Intervention, Substance Use Disorders, De-Escalation and Grief and Loss. Marla spent 11 years managing an HIV clinic for injection drug users, and a number of years as a hospice volunteer. She is now enjoying her own business providing contract consultation and training at clinics, service agencies and departments of health around the country. She is a proud founding member of the Professional Association of Social Workers in HIV/AIDS.
